JOB SUMMARY:
The Occupational Therapist in the Health Connect Program will be responsible for providing preventive care and monitoring chronic health conditions for clients in a supportive housing environment. This role involves conducting health assessments facilitating community and social integration and connecting clients to appropriate resources. The ideal candidate will utilize a traumainformed and personcentered approach to enhance the clients ability to live independently while managing their mental health substance use recovery and preventive healthcare needs.
ESSENTIAL TASKS:
- Conduct comprehensive health assessments for new and existing clients focusing on their psychiatric medical and social needs.
- Monitor and support clients with chronic health conditions ensuring they receive appropriate care and
- Develop and implement individualized care plans that promote independence and community
- Collaborate with the multidisciplinary team including LCSWs RNs social workers and peer specialists to provide holistic care.
- Provide training and education to clients and staff on healthrelated topics focusing on prevention and management of chronic conditions.
- Participate in the Clinical HighRisk Framework to assess and monitor clients needs identifying priority levels and ensuring timely interventions.
- Engage with clients to encourage program compliance and participation in healthpromoting
- Liaise with external healthcare providers and community resources to enhance client care and
- Document all assessments interventions and progress in compliance with program guidelines and
- Teach and reinforce daily living skills (IADLs) such as home management budgeting meal preparation and grocery shopping.
- Assist clients in developing routines that promote stability and independence
- Help clients access and navigate community resources including public transportation healthcare facilities and social services. Promote social engagement and community participation to enhance clients support networks.
- Participate in the Clinical HighRisk Meetings weekly to assess and monitor clients needs identifying priority levels
and ensuring timely interventions based on Clinical HighRisk Framework. Review and monitor Clients progress. - Other assigned duties.
ESSENTIAL KNOWLEDGE SKILLS AND ABILITIES:
- Ability to manage multiple projects and check on the completion of assigned
- Excellent Microsoft Suites(i.e. word document excel and power point)
- Strong interpersonal skills emotional intelligence and flexibility
- Proficient in computers and Electronic Health Record (EHR)
- Skilled at data management evaluation and improvement
- Ability to work independently and collaboratively with other team members
- Conduct comprehensive health assessments and develop individualized care

QUALIFICATIONS AND EXPERIENCE:
- Masters degree in occupational therapy from an accredited
- Licensed as an Occupational Therapist in the state of New York
- Experience working with individuals with serious mental illness (SMI) and substance use
- Strong understanding of traumainformed care and personcentered
- Ability to work collaboratively within a multidisciplinary
- Excellent communication and organizational
- Commitment to promoting independence and quality of life for individuals in a supported housing
Preferred Qualifications:
- Experience in supportive housing or communitybased settings (i.e. scatteredsite model).
- Knowledge of chronic disease management and preventive healthcare
- Familiarity with the New York State Office of Mental Health regulations and programs.
