Job Description
Overview
Function as an active member of the multidisciplinary team in planning appropriate treatment and continuing care plans for patients. Provide psychosocial evaluations has knowledge of psychosocial issues affecting the families of seriously ill neonates/infants and patients respond to referrals screen all highrisk patients teenage mothers and suicidal/homicidal patients for discharge planning and referrals. Assist the patient/family in choosing/utilizing appropriate services.

Qualifications
Graduate from an accredited school with a Masters Degree in Social Work or related field including Psychology Sociology or Marriage and Family Therapy. One year experience in the acute care setting is preferred.
