Description

Hennepin Health is seeking two Staff Nurses to join the Special Needs BasicCare (SNBC) team to provide care coordination for members who are certified disabled. These critical roles will offer care coordination and transition support to members with a multifaceted range of medical needs including establishing primary care accessing specialty care and facilitating other supportive services.

These positions will work within an interdisciplinary team of Social Workers and Community Health Specialists to sensitively address the social determinants of the health of SNBC members. These roles will focus on a vulnerable population to address their needs in a trauma informed and membercentric way. These are communitybased interactive positions often calling for creative solutions to difficult problems that are outside of traditional clinicbased interventions. The individuals filling this role must be flexible creative and understand marginalized populations specifically the unique health conditions affecting them and the social context in which they develop.

Location and hours:
These positions are hybrid and will be performed both onsite at the Health Services Building 525 Portland Avenue South 8th floor Minneapolis MN 55415 in the field and communitybased settings and remote as job duties require. Work hours will be Monday through Friday with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

About these position types:
These are fulltime benefit earning mobility assignments.

You must be a current regular or probationary employee of Hennepin County to be eligible for these positions. These positions are job mobility assignments with participation for up to 12 months with the possibility to extend up to 18 months. A mobility assignment is a voluntary job change to adifferent departmentthat is filled by a regular or probationary employee to carry out the duties of the same or a different job class.Hennepin County mobility assignments allow employees to develop a particular expertise and prepare for promotional opportunities as well as facilitate support and accelerate knowledge transfer across departments. Job mobility assignments are temporary; employees typically return to their original department at the conclusion of their mobility assignment.

In these positions you will:
  • Facilitate annual completion of the health risk assessment evaluate member needs; develop customized care plan reassessments; provide appropriate follow up to members. Conduct this work in the field community settings and telephonically to accommodate member needs.
  • Manage caseload of SNBC members with varied disabilities and behavioral health needs; providing responsive follow up and management of individualized care plan; work to address identified barriers conduct quarterly checkins with members
  • Work within MnChoices and HealthEdges Guiding Care (a care management system) to process assessments care plans and other relevant care coordination forms; prepare and maintain records including operational regulatory and utilization reports adhering to all contractual requirements specified by the Minnesota Department of Human Services (DHS).
  • Read and interpret client/patient charts lab reports and physician notes. Maintain working knowledge of the healthcare system including specialty services and programs that effectively address complex needs of members while incorporating an understanding of the impact the members housing economic status and other living conditions has on their health.
  • Participate in team member and all staff meetings; establish and maintain effective relationships with other SNBC care coordination waiver case management staff to effectively coordinate member care.

  • Public Health Nurse (PHN) certification.
  • Experience:
    • Managing a caseload within a team providing case management or care coordination services in a clinical or communitybased setting.
    • Working with Minnesota Health Care Programs.
    • Supporting racial equity or disparity reduction work from a healthcare perspective.
    • Using electronic health records MnChoices Microsoft Office Suite particularly Teams Excel and other related computer applications.
  • Knowledge of:
    • Principles and practices of care coordination; service referral within a county system.
    • Information systems relevant to care management coordination and reporting.
    • Trauma informed and harm reduction approaches to care.
  • Ability to:
    • Work independently in various settings supporting individuals who are experiencing marginalization and distress.
    • Act in an advocacy role on behalf of members.
    • Interpret and communicate verbal information on a onetoone basis and communicate complex medical and technical information to physicians members nursing staff and health care team.
    • Maintain accurate records of client/patient visits.
