Job Description

Occupational Therapist Inner Melbourne Metro Calvary Health Care Bethlehem Statewide PND Service

About the role

Based at Calvary Kooyong in Caulfield South Calvary Healthcare Bethlehem is a leading provider of specialist services for people living with progressive neurological diseases and those needing palliative care. Services include an inpatient ward Community Palliative Care Service State Wide Progressive Neurological Diseases Service CHSP and NDIS Provider service.

The site has state of the art brand new facilities including a cafe onsite. There are trains buses and shops within walking distance of the hospital. Free parking onsite.

You will work primarily in the Statewide Progressive Neurological Diseases Service (SPNDS) providing outpatient specialist support through hospitalbased or telehealth modes. SPNDS is a level 5 specialist service offering support to people in Victoria as well as in parts of SA NSW and Tas.

You will provide expert OT advice to people with progressive neurological diseases (PND) and their formal and informal support network to support living well. You are part of a supportive and compassionate multidisciplinary team including a wide range of allied health professionals neurologists and other specialists specialist nurses and external stakeholders. The caseload includes people with Motor Neurone Disease Huntingtons Disease and other Neuromuscular disorders and atypical Parkinsons Diseases at various stages of their disease trajectory. The clinic manages people from diagnosis until the terminal phase of their illness.

Your main duties will include:

  • Occupational Therapy assessment and intervention through direct and indirect modes
  • Attending case conferences
  • Supporting NDIS access and navigation of NDIS and aged care systems
  • Working with internal and external stakeholders to provide compassionate care
  • Supervision of grade 1 OTs AHAs or students as directed
  • Participation in OT department meetings and quality improvement work
  • Participation in SPNDS meetings and projects
  • Opportunities for presenting at conferences and education sessions
  • Participation in research projects as appropriate
  • Communication and collaboration with external allied health providers for patient management and to provide education about PND management
  • Administrative functions including keeping statistics and managing appointments
  • Documentation of patient notes and reports

About you

  • An OT who enjoys supporting quality of life and problem solving creative solutions
  • You have some experience or interest in developing skills in progressive neurological OT
  • You have outstanding communication skills
  • Thrive in a dynamic environment
  • Outstanding team player for best patient outcomes
  • You are selfdirected organised enjoy a balance of team and autonomous tasks
  • You are reflective in your clinical practice
  • You are a personcentred creative and flexible therapist who is able to work with dignity of risk and respect for individual choice and control
  • You have some knowledge in navigating the interface of palliative care health and disability to support patients and team members with coordination of care
  • AHPRA registered Occupational Therapist
  • Drivers Licence
