Job Description
Job Summary:
Abernathy Home Care is seeking a skilled and compassionate LPN (Licensed Practical Nurse)with Bilingual proficiency (preferred but not required)to join our team as a Home Health Aide Supervisor. This is a parttime individual contributor role located in Stoneham Massachusetts. The LPN will be responsible for providing guidance supervision and training to our home health aides to ensure the delivery of highquality care to our patients. The ideal candidate will have a passion for helping others and a desire to make a positive impact in the lives of individuals in our community.
Compensation & Benefits:
This is an hourly position with a compensation range of $35 to $39 per hour paid biweekly. Abernathy Home Care also offers a comprehensive benefits package that includes medical dental and vision insurance paid time off and opportunities for professional development.
Responsibilities:
Train supervise and evaluate home health aides to ensure compliance with industry standards and state regulations
Coordinate with other members of the healthcare team to develop and implement individualized care plans for patients
Monitor and report any changes in patients conditions to the appropriate healthcare professionals
Conduct regular inhome visits to assess patients needs and provide handson care as needed
Educate patients and their families on care plans and assist with any necessary medical equipment or supplies
Maintain accurate and uptodate documentation of patient care and progress
Requirements:
Current LPN license in the state of Massachusetts
Minimum of 1 year of experience working as an LPN in a healthcare setting preferably in home health or hospice care
Excellent communication and interpersonal skills
Ability to work independently and prioritize tasks effectively
Compassionate and empathetic personality with a strong desire to help others
CPR certification is preferred
