This will be a PartTime weekend shift position
Looking for an RN to serve Clark County and Northern Montgomery County
Pay Range:$33.0041.00/hour base based upon experience
Pay rate also increases based upon clinical assessment type and point system.
Competitive mileage reimbursement and cell phone stipend
Position Summary:
Responsible for assessing planning implementing and evaluating the nursing care for a caseload of clients from admission to discharge. Coordinates and supervises the nursing care delivered by the home health aides working under the Administrator of Health at Homes supervision. Assumes Case Management Responsibilities as assigned.
Job duties:
- Coordinates processes to support clients medication administration based on individual need.
- Assesses for significant changes in a clients condition and updates care plan accordingly.
- As assigned supervises the Home Health Aide in the home
- Initiates and completes all necessary documentation and reporting necessary to support services and charges.
- Communicates all order and status changes to the physician.
- Maintains a safe environment for clients and employees while exhibiting a positive attitude towards all customers.
- Initiates and maintains individual care plans based upon comprehensive assessment.
- Attends in services training staff meetings care plan conferences and seminars as scheduled.
- Responsible for completing nursing notes according to Medicare/Medicaid guidelines and Health at Home charting guidelines.

Qualifications:
- valid drivers license and safe driving record
- Registered Nurse from an accredited school of nursing with a current Ohio license.
- One yearof home health care experience preferred.
