JOB TITLE: Intensive InHome Specialist
LOCATION: Onsite
City/Stat/County: Wilmington NC New Hanover County
MINIMUM REQUIRED EDUCATIONAL/EXPERIENTIAL QUALIFICATIONS:
- Completion of Bachelors Degree within Human Service field preferred.
- Meets requirements under 10A NCAC 27G.0104 as an Associate or Qualified Professional for Mental Health.
GENERAL STATEMENT OF DUTIES:
Performs therapeutic interventions in the home and community settings with the identified youth and their family. The staff will work with a minimum of two other staff on the intensive inhome team to deliver this enhanced service.
POSITION HIGHLIGHTS:
- Provide direct services through community agencies and inhome services to support family preservation.
- Deliver oneonone interventions such as skill building activities intervene with positive behavioral supports enhance consumers knowledge of interpersonal and community relational skills.
- Willing to deliver services in a wide range of community settings such as detention centers schools homes jails libraries homeless shelters and any other applicable locations deemed appropriate and safe.
- Be available as part of a three person team to ensure 24/7/365 crisis response. Facetoface crisis management must be available in no less than two hours.
- Perform ongoing assessment and evaluation of the social and daily living needs of clients to ensure seamless transition from this enhanced service to the next medically necessary service.
- Provides supportive and informational counseling to include crisis intervention case management and the development of collateral contacts.
- To have a network of referral agency contacts and resources in order to fulfill the socioeconomic needs of the clients.
- To assist clients with housing employment social legal transportation and educational needs by connecting them with community services.
- Completes NCTOPPS with consumer if applicable.
- Is subject to both inside and outside environmental conditions and exposed to hazards such as fumes odors dusts mists gases or poor ventilation and may require the employee to work in close quarters.
- May be exposed to human body fluids and is subject to the OSHA requirements on blood borne pathogens.
ADDITIONAL INFORMATION:
- Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
- Considerable knowledge of mental health and substance abuse disorders.
- Considerable knowledge of available community resources services and the coordinating efforts needed for client with mental health and substance abuse problems.
- Considerable knowledge of treatment modalities for mental health disorders.
- Some knowledge of the principles and practices of substance abuse treatment and education activities.
- Ability to respectfully represent the agency and establish a rapport with the community.
- Ability to determine appropriate community services for clients and interface with community resources and agencies to meet client needs.
- Ability to communicate effective in oral and written forms.
- Ability to network and coordinate efforts between outreach treatment and other support systems for the clients.
- Ability to work well on a team and coordinate efforts to meet the needs of the client.
- Must be able to physically perform the basic life operational functions of climbing stooping crouching reaching pushing pulling lifting standing walking fingering grasping talking and hearing.
- Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift objects.
- Must posses the visual acuity to perform extensive reading and determine the accuracy of work assigned.
- Must have valid drivers license and be able to work in community settings.
