Central Ohio Primary Care is seeking a full time Advanced Practice Provider (APP) at our Same Day Center locations throughout Columbus OH.
The COPC Same Day Center (SDC) is our organizations internal outpatient acute care facility. All patients are scheduled by appointment services include anything from convenient supportive care to extended care visits with IV therapy. Imperative to this role is the ability and willingness to start and monitor the IV therapy. The SDC acts as an extension of the primary care physicians office. All patients are already established with COPC. SDC APPs work closely with physicians to appropriately manage a range of conditions to handle acute medical issues with follow up provided by the patients primary care provider.
Our centers are open MF 8am8pm and 84 on weekends. This is a fulltime position with various shifts available.
Essential Functions and Responsibilities:
- Obtains a compete health database that includes health history physical exam and any screening or diagnostic tests.
- Interprets health data by identifying risk factors and any variation from norms and/or diagnosing actual or potential health problems.
- Manage well and illness needs according to reasonable standards of care or guidelines.
- Evaluates the process and outcome of the management plan and modify the plan as indicated.
- Uses Universal Precautions when tasks being performed to present the possibility of exposure to blood and bodily fluids.
- Always maintains patient confidentiality.
- Documents all pertinent information accurately legibly and in a timely manner.
- Has knowledge of the clinic interventions and equipment necessary to meet the specific needs of the patient population served.
- Collaborate with both Supervising Physician and Care Team with flexibility openness and patient centered mentality.
Qualifications
Education Licensures & Certifications:
- Masters Level Degree.
- Must be considered a Certified Nurse Practitioner (CNP) or Physician Assistant (PA) by nationally recognized board in state of Ohio.
- Experience working in an acute care Inpatient ED or Urgent Care setting as a Certified Nurse Practitioner (CNP) or Physicians Assistant preferred.
Knowledge Skills & Abilities:
- Ability to start and monitor IV therapy
- Proficient in clinical practice protocols and delivering ongoing care in chronic disease management acute visits and wellness visits
- Proficient in Microsoft Office applications
- Extensive knowledge of medical terminology
- Excellent relationship building skills
- Excellent listening communication and documentation skills
- Knowledge of Principles practices and procedures used in the detection diagnosis and treatment of common health problems
- Knowledge safety and infection control practices and procedures
- Strong problemsolving skills
- Possesses customer serviceoriented mindset
- EPIC experience is helpful. Training is available.
