Job Description

Are you passionate about supporting people to live well through personalised care and rehabilitation

We are seeking a compassionate and proactive Rehabilitation Assistant to join our multidisciplinary team supporting patients across our InPatient Unit Community Services and Living Well Centre. This role is a fantastic opportunity to be part of a valuesled hospice helping people to maintain independence and quality of life at every stage of their journey.

As a key part of our team you will:

  • Deliver onetoone and group rehabilitation support under the guidance of a qualified therapist.

  • Promote independence through personalised activity and mobility programmes.

  • Support patients with daily living tasks and exercise plans in a compassionate and empowering way.

  • Work with families and carers to build confidence and understanding in rehabilitative approaches.

  • Help patients and carers navigate equipment needs and safe usage at home.

  • Contribute to holistic multidisciplinary care planning across the hospice.

This is a handson role ideal for someone with a passion for helping others strong communication skills and an interest in rehabilitation within palliative care.

Job Details:

  • Location: St Peter & St James Hospice BN8 4ED

  • Salary: Band 3

  • Working Hours: Parttime hours 30 hours per week

  • Contract Type: Permanent

  • Salary: per annum FTE

Key Responsibilities:

  • Support patients in achieving their individual rehabilitation goals.

  • Assist with physiotherapy programmes and exercises including breathlessness management transfer practice and walking sessions.

  • Provide emotional support and encouragement to patients and carers.

  • Carry out home visits and environmental risk assessments.

  • Contribute to group sessions in the Living Well Centre.

  • Maintain accurate patient records outcome measures and risk assessments.

  • Work collaboratively with nurses medical staff supportive care and volunteers.

  • Champion patient independence and selfmanagement strategies.

What Were Looking For:

To thrive in this role you will have:

  • NVQ Level 3 in Health & Social Care or equivalent experience in a therapy care or hospice setting.

  • Knowledge of care or rehabilitation environments.

  • Understanding of the physiotherapy role and palliative rehabilitation.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and manage your own caseload.

  • Strong recordkeeping and computer skills (System One Microsoft Office).

  • A positive proactive and compassionate approach.

Desirable (but not essential):

  • Previous experience in a Band 3 rehab/therapy assistant role.

  • Experience working in group sessions.

  • Own vehicle for community visits.
