Advanced Practice Provider PANP - Acute Care Surgery Nocturnist

Position Description:

The Medical College of Wisconsins Department of Surgery Division of Trauma and Acute Care is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures located in Menomonee Falls WI at Community Memorial Hospital.

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration patient care patient safety research quality improvement and teaching.

The Ideal Candidate will be experienced in acute or critical care with a strong foundation in evidencebased practice and ensures an environment of safe quality care wanting to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include but not be limited to providing direct patient care to an adult population in an inpatient setting.

In this role you will:

• Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring surgical intervention in inpatient environment as a member of the Acute Care Surgery team.
• Writes accurate and timely progress notes procedure notes and/or admission and transfer orders.
• Order perform and interpret screening and diagnostic procedures and tests and appropriately communicates and documents information.
• Privileged to perform procedures beyond core as identified by the acute care surgery services.
• Facilitates and communicates with patient family and staff to promote continuity of care across the continuum of care.
• Attend required training and meets key regulatory requirements including those of the privileging institution CMS and the Joint Commission.
• Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
• Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field through educational programs inservices professional associates and current literature.
• Attends daily rounds with attendings and Acute Care Surgery physician and APP team presents 24hour patient report.
• Acts as a liaison for communication of care plans and discharge destinations from ICU to floor teams. Attends care coordination rounds as able.
• Participates in Acute Care Surgery performance improvement activities to ensure the quality of care and effectiveness of service.
• Completes complex wound care and negative pressure wound therapy.
• Provides education and facilitates communication between surgical residents and medical students as required.
• Assistance in the operating room as needed.
• Complete consultations as required from referring teams.

Core Advanced Practice Responsibilities:

• Perform comprehensive histories and physical exams identifies normal and abnormal findings including physical cognitive and psychosocial status.
• Order perform and interpret screening and diagnostic procedures and tests and appropriately communicates and documents information
• Write accurate and timely progress notes procedure notes and/or admission and transfer orders
• Develop pharmaceutical and nonpharmaceutical treatment plans.
• Monitor effectiveness of plan of care
• Educate patients on disease and treatment plans.
• Perform procedures as trained and appropriately credentialed.
• Document within the Electronic Medical Record (EMR) and completes other necessary documents.
• Collaborate and/or consults with physicians and other health care providers as appropriate.
• Educates counsels and assists patients and families regarding current health problems health maintenance activities and preventive behaviors.
• Attend required training and meets key regulatory requirements including those of the privileging institution CMS and the Joint Commission.
• Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field through educational programs inservices professional associates and current literature.
