Job Description

Supervisor of Clinical Care Transition Services
Kenneth Young Center is a 501(c)3 notforprofit dedicated to providing comprehensive outpatient behavioral health services to individuals of all walks of life. Job Scope: The supervisor provides program and clinical oversight to a team of Clinical Care Managers serving adults living in nursing facilities (NF) and specialized mental health rehabilitation facilities (SMHRF) to support their transition to independent living as part of the Comprehensive Transition Grant serving Colbert & Williams Class Members/clients. Team follows adults during transition process and provides 18 months of followup monitoring.
Program Responsibilities
  • Oversee overall operations for provision of Clinical Care Manager services by a team to Williams and Colbert members being transitioned out of nursing homes and SMHRF (Specialized Mental Health Rehabilitation Facilities) in Northwest Suburbs of Chicago and Lake County
  • Provide clinical and administrative supervision of up to 10 direct reports and provide regularly scheduled and adhoc individual and/or group supervision to direct reports
  • Coordinate with other Supervisor of Clinical Care to provide support coverage and coordination
  • Monitor program staffing needs and participate in hiring orientation and training of new staff
  • Complete performance evaluations building on strengths and identifying ongoing growth/training needs and provide ongoing training and development
  • Accept referrals and coordinate caseloads for direct reports
  • Monitor individual productivity goals grant deliverables and oversee performance to ensure team meets goals and follows program policies and deliverables
  • Provide inperson activities to Class Members to support direct reports thru training evaluation and coverage of care manager duties
  • Ensure that care provided integrates all facets of clients lives including mental/emotional health physical health/wellness employment and education social and family relationships substance use/abuse issues spiritual communityintegration and housing needs
  • Communicate clearly with interdisciplinary team including nursing facility staff Case Managers Integrated Health and Aftercare supports Class Member supports and providers to ensure working together on service goals
  • Ensure accurate identification and timely submission of all incident reports
  • Monitor team activities to ensure meeting grant deliverables and ensure that all clinical activities performed by the team are compliant with standards policies and procedures of the organization
  • Assist with completion of daily data reporting weekly monthly and quarterly reports with timely submission to DHS/DMH as needed
  • Oversee all expenditures to ensure staff are following internal processes meeting grant/contract requirements and following fiscal responsibility
  • Coordinate with other team and department supervisors and managers to ensure referral to resources and connection to services such as SUPR Employment Individual Therapy Case Management and comprehensive mental health aftercare (ACT CST and CSI) services
Individual Responsibilities
  • Meet regularly with Class Members at assigned nursing facilities in Chicagoland focusing on Northwest Suburbs
  • Manage own limited caseload and provide direct Clinical Care Management activities
  • Take initiative and be accountable for and manage all aspects of transition including being able to navigate changes at the system or individual level and advocate for Class Members and resolve conflicts
  • Create comprehensive wraparound service plans for class members using input from Class Members SMHRF/NF staff family and guardians while focusing on a Class Members needs desires and preferences
  • Provide posttransition monitoring to Class Members for 18 months and meet minimum inperson visit schedule to ensure all services are being accessed and delivered
  • Complete transition checklist to ensure all required activities have been completed
  • Participate in clinical/administrative supervision; seek individual consultation with Manager when needed.
  • Complete all clinical documentation and required reports in an accurate and timely manner
  • Interpret and communicate at the service delivery level Center policies and procedures
  • Participate in orientations all staff trainings and training and development seminars
  • Submit in a timely and accurate fashion schedules timesheets mileage and other reports required by the Centers policies and procedures
Minimum Position Requirements
  • Masters Degree from an accredited college or university in social work psychology counseling or a related discipline
  • Preferred Licensed in Illinois as LPHA (Licensed Practitioner of the Healing Arts) LCSW (Licensed Clinical Social Worker) or LCPC (Licensed Clinical Professional Counselor). Will consider candidates with Qualified Mental Health Professional (QMHP) designation who are working towards LPHA/LCSW/LCPC with extensive experience.
  • Preferred 2 years of experience working with adults with serious mental illness and/or chronic health conditions or complex physical health and mobility needs
  • 1 year experience working in nontraditional treatment environments i.e.inhome residential nursing facility hospital community settings
  • Preferred 1 year of experience providing administrative and clinical supervision
  • Applicant must have a valid IL Drivers License proof of insurance and daily access to a reliable well maintained vehicle
  • Preferred knowledge of Olmstead Colbert and/or Williams Consent Decrees
  • Basic computer internet and smartphone proficiency including electronic communication platforms and knowledge of Word and Excel
Schedule
  • Fulltime Salary Exempt 37.5 hours per week
  • Schedule is weekday business hours MondayFriday 9am5pm or comparable
