Job Description
Tidewell Hospice a member of Empath Health is currently seeking a compassionate caring and committed Admissions Registered Nurse (RN) to join our team!
Saturday/Sunday/Monday 9am9:30pm . This is primarily Charlotte and Desoto Counties in the field setting with the ability to travel throughout the entire service area including the South and Central hospitals. Will also be cross trained in the hospital for South and Central territories
The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care while advocating for optimal access to the right care at the right time in any care setting. The Admissions RN completes the admission process based on care program specifics guidelines and patientspecific needs. The RN exhibits passion for the services we offer also while engaging in mission support and portrays excellence in customer service and relationship building internally and externally.
The successful applicant will coordinate patient admissions in patient homes longterm care facilities hospitals assistedliving facilities and hospice houses.
Position Requirements:
- Education and/or Experience: Associates degree (A.A. or equivalent from twoyear college or technical school; Minimum of or six months to oneyear related experience and/or training; or equivalent combination of education and experience.
- Licensure:
Valid Florida license as a Registered Nurse
Valid Florida State Drivers License - For fieldbased positions employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance a copy of which will be placed in the employees HR file.
