Job Description

TNC Community is looking for people who want to make a difference. Do you have a passion for helping and caring for others If so we are currently accepting applications for our Health Support Coordinator position providing support and care for individuals with Intellectual Developmental Disabilities in our group homes and supported living sites throughout the Kansas City metro area.

Health Support Coordinators work as a part of the health team and assist TNCs Community RNs and Programs team to ensure appropriate and current health care is provided for the individuals residing in the group home and ISL environments. This position primarily schedules and provides transportation to health appointments for TNCsupported individuals according to their own individualized Health Maintenance Schedule which is updated annually. The HSC will also schedule and manage any additional appointments as recommended or needed to provide optimal health care support for each individual.

DESIRED EDUCATION AND EXPERIENCE

Must be at least 18 years of age.

High School Diploma or equivalent or commensurate experience

Certification as CMT or CNA preferred

Minimum 1year experience working with individuals with intellectual and/or developmental disabilities

The position requires sufficient communication skills: written verbal and nonverbal to effectively engage with individuals staff and administration

MINIMUM REQUIREMENTS

Ability to clear background checks and drug/alcohol screenings as required.

Current CPR/First Aid certification (training will be provided if necessary)

Maintain current Abuse/Neglect Training (training will be provided)

Completion of Gentle Teaching or Equivalent (training will be provided)

Must participate in all mandatory trainings and recertifications as required.

Ability to drive motorized vehicles

Must obtain a class E Missouri drivers license or equivalent within the first 10 days of employment

Ability to lift 75 pounds (weight may vary)

Flexible schedule

Be familiar with and able to utilize a computer

Salary: Up to $18.39 per hour
