Job Description

Position Description

We are seeking a compassionate skilled and dedicated Juvenile Justice Clinical Coordinator tojoin our team in providing holistic care and coordination for youth involved in the juvenilejustice system. This role is designed for a clinical professional with a strong understanding ofmental health the effects it has on behavior and a passion for ensuring that youth cansuccessfully reintegrate into their communities. The Clinical Coordinator will assess individualneeds build interdisciplinary teams and create care plans focused on healing and growth ratherthan punishment for children who may be released from detention.


Duties and Responsibilities:

  • Conduct multifaceted needs assessments as juvenile justice cases come in identifying both immediate and longterm needs to support youth in their reintegration into the community.
  • Develop and lead an interdisciplinary team based on the needs of each client including social workers mental health professionals immigration lawyers education specialists and other communitybased service providers.
  • Ensure the clients needs are addressed from a holistic perspective including mental health social services legal assistance and educational support.
  • Advocate for youth who have entered the juvenile justice system framing their involvement in the system as a result of underlying needs rather than criminal intentions.
  • Collaborate with juvenile justice psychologists or communitybased clinicians to facilitate comprehensive psychosocial evaluations and ensure case plans are developed based on evaluation results.
  • Provide regular insights on how to support the healing process in the community instead of relying on continued detention or punitive measures.
  • Track clients out on PAT ensuring that progress is monitored and documented to demonstrate successful community reintegration and behavioral growth.
  • Provide direct supportive counseling and checkins with clients and their families especially in cases with high lethality or significant mental health needs.
  • Facilitate communication and collaboration between juvenile justice teams and external services to ensure clients receive a continuum of care.
  • Monitor the outcomes and effectiveness of care plans and services making necessary adjustments to improve youth outcomes.
  • Regularly assess and manage risks related to clients safety and wellbeing ensuring that all involved parties are informed and proactive in providing the needed care.
  • Serve as an advocate for youth in various settings including court proceedings meetings with service providers and family conferences.
