Support Worker Youth Disability Aged Ipswich/Toowoomba

From its humble beginnings supporting the Health and Community sector in Sydney we have expanded to deliver workforce services across Australia that support the sustainability and quality service delivery for our workforce partners.
Currently we are building our workforce pools across the Ipswich and Toowoombaareaso we can provide support to our evergrowing client base in the region. Duties include:
- Build and maintain positive relationships with the people you support.
- Follow established care plans.
- Collaborate with relevant stakeholders.
- Respond to crisis situations offering immediate support and collaborating with appropriate authorities.
- Maintain accurate records of interactions assessments and program outcomes.
- Work positively and collaborate with team members to ensure the highest care and support provided.
Requirements:
YOUTH
- Cert IV in Child Youth and Family Intervention or other relevant field (eg: Disability Mental Health etc. Or working toward
- Hope & Healing Certificate (or willing to obtain)
AGED DISABILITY
- Cert III in Individual support or another relevant field.
ALL
- Current First Aid and CPR (or willing to obtain)
- Recent National Police Clearance
- Current NDIS Worker Screening (or willing to obtain)
- Current (Paid) Working with Children check (or willing to obtain)
- Recent and relevant experience in a similar role
- 2 professional references
- Unrestricted Australian Drivers License (required if providing community access support driving facility vehicles)
- Comprehensive Car Insurance (or willing to obtain required if using own vehicle for work purposes)
Drivers licenseis Required.
