Job Summary
The Physical Therapist (PT) provides highquality patientcentered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans that focus on achieving goals promoting patient engagement and ensuring a positive patient experience. The Physical Therapist provides or directs the majority of the patient s physical therapy to support patients throughout the continuum of care.
Job Responsibilities
- You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
- You will provide purposeful and skilled therapeutic care at the patient s home using procedures and techniques to produce a change in condition/function consistent with the physician s plan of care.
- You will instruct the patient/family/caregiver in the use of therapies exercises therapeutic appliances and/or prosthetic devices to be used outside of therapy sessions.
- You will prioritize caseload determine daily staffing needs and coordinate assignments/handoffs.
Qualifications
Basic Cardiac Life Support must be obtained within 30 days of the start date
Licensed Physical Therapist in the state of FL REQUIRED
Masters Degree or Bachelors Degree
MUST HAVE AT LEAST 2 YEARS OF RECENT ACUTE CARE EXPERIENCE
