Job Description
Join a dynamic team dedicated to providing exceptional home healthcare.
We are looking for a licensed and compassionate Physical Therapist to join our this role youll provide in-home therapy services that support patients rehabilitation mobility and quality of life.
Role Overview:
As a Physical Therapist you will assess plan and deliver therapeutic interventions for patients recovering from injury illness or surgery. Youll help patients regain mobility manage pain and safely perform daily activities all from the comfort of their homes.
Responsibilities:
Conduct initial and ongoing assessments of patients physical condition and functional abilities
Develop personalized care and therapy plans in collaboration with the care team
Provide direct physical therapy services including strength balance gait and mobility training
Educate patients and caregivers on exercises safety and equipment use
Monitor patient progress adjust care plans and report outcomes
Recommend assistive devices and home safety modifications as needed
Collaborate with nurses occupational therapists aides and social workers for coordinated care
The Ideal Candidate:
Current Physical Therapist license
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Minimum of 1 year experience in home health rehab or hospital setting preferred
Strong clinical communication and assessment skills
Able to work independently and manage a mobile caseload
Valid drivers license and reliable transportation required
CPR certification and compliance with health requirements
