Job Description

We are seeking a highly motivated and dedicated Clinical Pharmacist Specialist to join our healthcare team specializing in Internal Medicine Ambulatory Care. This role is critical in providing safe effective and timely medication therapy management for patients in the ambulatory care setting focusing on transitions of care chronic disease management and polypharmacy. The pharmacist will be responsible for performing comprehensive medication reconciliation resolving discrepancies and collaborating with healthcare providers to optimize patient outcomes. Additionally the pharmacist will play a key role in the expansion of pharmacydriven services and contribute to the billing of services to support sustainable clinical initiatives. The ideal candidate will bring expertise in ambulatory care clinical pharmacy and billing practices for pharmacy services.

QUALIFICATIONS

  • Highly motivated selfdirected pharmacist with a strong commitment to patient care.
  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Active pharmacist license in Colorado or the ability to obtain one within 60 days of hire.
  • Completion of ASHPaccredited PGY1 Pharmacy Residency; PGY2 specialty residency preferred.
  • 24 years of clinical pharmacy experience with a focus on ambulatory care internal medicine or transitions of care preferred.
  • Experience with ambulatory care billing for pharmacistprovided services is highly desirable.
  • Extensive knowledge of pharmacotherapy drug interactions and medication reconciliation processes.
  • Strong communication and collaboration skills with the ability to work effectively with multidisciplinary teams and patients.
