Job Description
Description of the role:
The Social Worker at Mission Community Hospital plays a critical role in providing support and assistance to individuals and families who are facing various social emotional and behavioral challenges. With a focus on improving the overall wellbeing of patients the Social Worker collaborates with the healthcare team to develop and implement effective intervention plans.
Responsibilities:
- Conduct comprehensive assessments of patients social and emotional needs.
- Develop individualized care plans and coordinate with interdisciplinary team members to ensure proper implementation.
- Advocate for patients and their families to access necessary resources and services.
- Provide counseling and support to promote positive coping strategies and behavioral changes.
- Collaborate with community organizations and agencies to facilitate referrals and linkages to additional support systems.
- Maintain accurate and uptodate documentation of all interventions and patient progress.
Requirements:
- Masters degree in Social Work (MSW) from an accredited program.
- Valid license or certification as a Social Worker in the state of California.
- Prior experience in a healthcare setting preferably in a hospital or medical facility.
- Strong knowledge of social work theories assessment tools and intervention strategies.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team and collaborate with different departments.
