Job Description
Hospice Social Worker
Broad ReachHospice is seeking a qualified SOCIAL WORKER to join our team of dedicatedhealthcare professionals.
As a Social Worker at Broad Reach Hospice you willcollaborate with our interdisciplinary team to participate in assessmentsdevelop care plans carry out psychosocial treatments and manage dischargeplanning for both inpatient and home care patients.
Schedule: PartTime 3 or 4 days per week.
Pay Range: $35 $45 per hour depending upon experience.
Social Worker Responsibilities:
Perform comprehensive initial psychosocial andbereavement evaluations and assist in the development and implementation of theplan of care.
Continually assess patient/family needs andcounseling.
Provide short term crisis intervention andindividual/family counseling.
Observe evaluate and report on psychosocialspiritualphysical and financial condition affecting the patient/family.
Develop and maintain communication withcommunity agencies/services to promote interagency cooperation and tofacilitate referrals.
Travel extensively to patient homes hospitalsand nursing facilitiesthroughout Broad Reach Hospices service area.
Social Worker Requirements:
Bachelors or Masters degree in Social Work.
Massachusetts State Licensure at LSW LCSW orLICSW level.
Strong background in Social Work
Experience in health services delivery.
Strong communication skills.
Strong team working skills.
Experience in conducting assessments ofpatient/family psychosocial history.
Strong problem solving and task prioritizationskills.
