Position: FullTime Branch:Enid OK Schedule: MF 85 Salaried

Your Responsibilities
As a Hospice RN Case Manager you will:
- Provide patient care across the continuum managing hospice services with empathy and expertise.
- Assess plan implement and evaluate individualized care plans for patients.
- Support patients and families through endoflife care with compassion and dignity.
- Collaborate with interdisciplinary teams to ensure seamless care delivery.
- Maintain accurate documentation and manage multiple priorities effectively.
What You Bring
- Education: Graduate of an accredited Certificate Diploma Associate or Baccalaureate Nursing Program.
- Licensure: Current Registered Nurse (RN) license in Insert State.
- Experience:
- Minimum of 1 year in a clinical care setting.
- Hospice or home health experience (preferred).
- Skills: Strong nursing skills ability to manage priorities and dedication to patientcentered care.
- Transportation: Reliable transportation to perform job responsibilities.
- Ability to sit stand bend lift and move intermittently with the capability to lift 50100 lbs.
