Patient Care Assistants

EXAR Healthcare Staffing Agency is looking for a Patient Care Assistant/Caregiver for a wellestablished Home Care Agency based in Indiana. This is a permanent placement position with the Home Care Agency.

This position provides oneonone supervision for safety and assistance to the client in activities of daily
living to maintain the client in their home. The Personal Care Assistant provides services to agency clients
involving personal cares transfer assist cleaning & other housekeeping functions errands companionship
and performs other duties as directed by the agency director. The Personal Care Assistant will be assigned
in a manner that promotes quality continuity and safety of a patient.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Administrative
1. Timely and accurate completion of visit/time record documentation and verification of service forms.
2. Adhere to the Health Insurance Portability and Accountability Act (HIPPA) privacy policies and procedures.
Maintain confidentiality of all information pertaining to clients families and employees.
3. Maintain positive working relationships with clients client families Kings Grace Homecare staff
community providers and referral sources.
4. Always maintain a professional appearance.
5. Participate as appropriate in all inservice and education programs required by the agency.

Services and Procedures
1. Assist with Toileting functions
2. Transfers and ambulation
3. Assisting with bathing grooming and hair washing necessary for personal hygiene
4. Provide medication reminders.
5. Dressing or undressing of the client(s)
6. Assistance with eating meal preparation and grocery shopping
7. Accompanying to medical appointments
8. Redirection monitoring and observation that are integral part of a personal service plan in the above
items
9. Redirection and intervention for behavior including observation and monitoring
10. Incidental household services that are an integral part of a personal service plan in the above items
11. Appropriate reporting of changes in client status.

Education and Experience
1. Eighteen 18 years of age
2. Be able to provide covered PSA services according to the persons PSA Service plan.
3. Meets the training requirements in accordance with state and federal laws.
4. Must be able to read and write in English.

Language Mathematical & Reasoning Skills
1. Excellent customer service and listening skills
2. Excellent oral and written communication skills; able to effectively communicate with the client & PSA
Provider Agency
3. Ability to read and write clear English.
4. Ability to compute basic mathematics.
5. Demonstrate the ability to make appropriate professional judgments.
6. Ability to handle multiple tasks.
7. Able to and provide covered PSA services according to the clients service plan respond appropriately to
clients needs and reports changes in the clients condition.
8. Ability to read and follow written instructions and document care given.
9. Strong organizational and problemsolving skills.
10. Selfdirecting with the ability to work with little direct supervision.
11. Empathy for the needs of the ill injured frail and the impaired.
12. Demonstrates tact patience and good personal hygiene.

1. Occasional use of hands wrists and fingers and lifting or moving up to 75 lbs. may be required.
2. Movements such as stooping crouching bending kneeling climbing and reaching are
occasionally required.
3. This position requires spending much of the workday standing and walking with occasional
sitting.
4. Noise and/or level of distractions in the work environment is moderate.
5. Specific vision abilities required by this position include close vision distance vision and
peripheral vision.
6. Access to a working telephone for regular communication with Agency Manager client
responsible party or Homecare / Staffing Coordinator.
