Scheduled Weekly Hours:
24Gundersen Health System is seeking a parttime RN Care Coordinator for our Hospice department. This is your opportunity to work primarily day shift while doing the work you love in the beautiful Coulee region.
What you will do:
0.6 FTE48 hours/every two weeks
Work primarily daytime hours occasional PM hours & 2 holidays/year (no weekends)
Occasional oncall coverage for evenings and nights
Provide compassionate care to families and patients thru endoflife transitions within a 50mile radius of La Crosse WI
Admit Patients to hospice function as a team lead/charge RN in office provide routine patient visits
Work with a multidisciplinary team consisting of nurses social workers chaplains bereavement coordinators and hospice aides
What you need:
Associate Degree in Nursing
Minimum of 1 year RN experience
RN licensure to practice in the state of Wisconsin upon hire
RN licensure to practice in the state of Minnesota within 3 months of hire
