The Transplant Social Worker provides holistic social work services to transplant candidates recipients living donors and their families through all transplant phases of care. This includes the date of referral transplantation and the duration of posttransplantation care. As an integral member of the transplant team the Transplant Social Worker provides emotional support and patient education addresses social determinants of health assumes responsibility for referrals to community agencies collaborates and coordinates patient care with other transplant team members and assists with concrete services. Through the use of a wide range of interventions and treatment modalities the Transplant Social Worker contributes to the achievement of optimal health satisfaction and costeffective care by addressing the behavioral social and environmental issues which may impede transplantation and posttransplantation care. The Transplant Social Workers duties include but are not limited to consultation through comprehensive assessment counseling interventions planning and referral of services to meet the psychosocial needs of patients and families. The Transplant Social Worker will round on assigned patients with the Transplant Team. Candidates must have excellent interpersonal and written communication skills be knowledgeable of available community resources have current clinical assessment skills document all interventions participate in patient and staff education outreach activities professional presentations transplant team care conferences and selection committee meetings quality improvement and/or research projects and other relevant duties as assigned. The provision of social work services is mandated by CMS for Medicareapproved transplant centers.
MSW from a Social Work program accredited by the Council on Social Work (CSWE) NYS Certification (LMSW or LCSW) and three years relevant postmasters experience are required.
Transplant experience (preferably in a hospital and/or healthcare environment) is preferred.Active participation in transplantrelated and/or professional social work organizationsis strongly preferred.
