As a Certified Community Behavioral Health Clinic CNS Healthcares mission is to serve the people of our communities. Our Case Aide Assistants are responsible for assisting our interdisciplinary teams under the direction of our case management team supervisors and case managers. They advocate for the individuals we serve and assist our case managers in providing personcentered services to help facilitate care. They assist the individuals we serve by linking them to benefit employment and community resources.

- Work with individuals served that have some form of mental illness substance use disorders or developmental disabilities.
- Assist Case Managers in providing personcentered services by educating individuals on psychosocial recovery principles.
- Maintain confidentiality at all times.
- Participate in team meetings as required.
- Transport individuals in agency owed vehicles or personal vehicles as assigned.
- Provide individual served support in the event of psychiatric crisis.
- Completion of individualized weekly/monthly home visits goal reassessments and updated paperwork and reporting requirements.
- Complete all necessary documentation according to program policies and procedures.
- Assist the individual served by linking to benefits such SSI Medicaid Medicare food stamps and workrelated community resources such as TANF funds.
- Assist the individual served in utilizing free vocational training and job referral services and housing maintenance services through referrals to appropriate community agencies.
- Assist individuals served with emergency situations and needs in collaboration with other community agencies.
- Assist support and advocate for individuals served in gaining access to community programs and services as they are indicated in the IPOS especially health care dental services selfhelp groups budgeting classes and life skills groups.
- Complete all necessary documentation within program time limits.
- Able to work independently with minimal supervision.
- Must be able to use computer applications (Microsoft Word Excel).
- Must be able to complete forms and reports as directed.
- Must possess strong organizational skills.
- Performs other duties as assigned
Qualifications:
- High school diploma or equivalent
- Valid and unrestricted Michigan Drivers License
- Reliable transportation and current proof of auto insurance
- Excellent customer service skills
- Technical keyboarding and Microsoft Office skills
- Ability to deal effectively with various levels of agency personnel outside customers and community agencies and groups
Preferred
- A minimum of 1 year of experience working in a mental health setting
- Group home or other related training
