40Summary:
The social worker provides behavioral health triage and intake assessment on site in primary care clinics and over the phone to determine appropriate level of care and treatment for established Neighborhood Clinic (NHC) patients. The social worker assesses patients and accompanying family systems for needed assistance with resource procurement that aids the familys efforts in caring for their childs behavioral health needs. The social worker provides ongoing assessment resource navigation case management limited supportive counseling and/or crisis intervention as appropriate for NHC patients referred to the behavioral health team. The social worker collaborates with the behavioral health therapists to support behavioral services within the NHCs.
Education & Experience:
Masters level social work degree required (LMSW) or (LCSW)
Three years of social work experience
Experience working in a healthcare setting and/or providing behavioral health services preferred Bilingual English and Spanish preferred

Certification/Licensure:
Social Work current unrestricted Texas license at the masters level (LMSW/LCSW)
