CORE JOB SUMMARY
The Patient Navigator (H) facilitates patients ability to obtain services by identifying barriers to care coordinating the development of treatment plans and monitoring the implementation and followup of services ordered by caregivers.
CORE JOB FUNCTIONS
- Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
- Ensures patients have access to insurance and social service programs for which they are eligible.
- Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
- Identifies and resolves real or perceived barriers to care.
- Educates and counsels patients on issues related to their health and life situations.
- Maintains current knowledge of community health welfare resources and any other programs available to patients.
- Handle patient communications and contact healthcare facilities for medical records.
- Address care gaps with the nurse navigator.
- Prepares tracks and processes medical records and pathology specimens. Reviews and file medical records for completeness; use tracking systems for accurate documentation.
- Provides administrative support to the department by answering phones address inquiries and direct calls or take messages Copy fax emailing and mailing records while ensuring confidentiality.
- Adheres to University and unitlevel policies and procedures and safeguards University assets.

CORE QUALIFICATIONS
- High School diploma or equivalent
- Minimum 1 year of relevant work experience
- Experience in a medical office or hospital preferred.
- Ability to process and handle confidential information with discretion
- Ability to communicate effectively in both oral and written form
- Ability to handle difficult and stressful situations with professional composure
- Ability to maintain effective interpersonal relationships
- Ability to understand and follow instructions
Department Specific Functions: (SDG) Navigation Program
- Telephone responsibilities include but are not limited to speaking to the patient and contacting a variety of health care facilities to obtain patient medical record information.
- Address barriers and gaps in care with the nurse navigator.
- Explains consent for obtaining outside records and HIPAA (if applicable) to patient; obtains signatures (may be done via email)
- Submits signed consent to health care centers as appropriate to initiate retrieval of medical records pathology slides xrays and any other pertinent report.
- Tracks pending records to ensure delivery prior to clinic visit such as pathology reports etc.
- Communicates to the appropriate SDG Nurse Navigator if difficult or unable to obtain a pertinent record.
- Travels within the facility and campus to retrieve records/films/CD/s as needed.
- Prepare pathology slides and submits for consultation.
- Maintains confidentiality of patient information.
- Prepares medical records charts and other relevant documentation.
- Requests tracks and processes pathology specimens accordingly.
- Review medical records for completeness and files documents appropriately.
- Pulls medical records as requested by nurse navigator.
- Uses tracking system and databases to accurately document placement of all medical records.
- Copies faxes emails and mails medical records while ensuring patient confidentiality.
- Answers telephones and responds to questions concerns and directs calls or obtains messages.
- Adheres to University and unitlevel policies and procedures and safeguards University assets.
Knowledge Skills and Abilities:
- Excellent customer service skills are required.
- Strong written and oral communication skills.
- Knowledge of medical terminology is preferred.
- Able to handle multiple tasks simultaneously.
Ability to establish and maintain effective working relationships with patients and staff
- Computer skills Microsoft Office
