Job Description
Overview

To provide case management & milieu management complete psychosocial assessments assist in the development and reevaluation of Treatment Plans and work as a member of an interdisciplinary team and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.
Qualifications

EDUCATION EXPERIENCE TRAINING
1. Bachelors degree Social Work Marriage and Family Therapy or other related required.
2. Masters in Psychology Social Work Marriage and Family Therapy Licensed Professional Clinical Counselor or related field preferred.
3. Current registration an intern with Board as appropriate to education and licensing requirements.
4. Current BLS certificate upon hire and maintain current.
5. Minimum one 1 year Inpatient l or Outpatient Behavioral Health experience preferred.
Saint Mary of Nazareth Hospital offers competitive compensation and a reasonable compensation estimate for this role is $36.39 to $36.39. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire in which a wide range of factors will be considered including but not limited to skillset years of applicable experience education credentials and licensure.
