Job Description
Pine Ridge is a 111bed Skilled Nursing Facility that is Medicare and Medicaid Certified with 33 private rooms and 39 semiprivate rooms. The facility sits on Hansen Care Parks beautiful wooded property with spacious shower and restroom facilities beauty salon and barber shop services walking paths state of the art rehabilitation areas for those who need physical occupational or speech therapy and cozy places to relax throughout the facility. We provide care for individuals who may require a variety of medical services including Shortterm Rehabilitation Longterm Care 24 Hour Skilled Nursing Hospice and Respite Care Restorative Care and Certified Wound Care.

The health system comprises threehospitals an outpatient surgery center a regional cancer center rehabilitation centers two longterm care residences home care and hospice services and 34 affiliate physician practice locations. We provide both inpatient and outpatientmedical servicesthroughout Southwest Michigan.
Scope of work
Responsible for accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) collaborating with Staff Nurses Nurse Aides and the interdisciplinary team on the assessment process and development of resident MDS/Care Plans. Position will monitor Medicare and Medicaid requirements.
Qualifications
Required
Associates in Nursing upon hire
2 years Nursing and MDS process experience with demonstrated nursing assessment skills
State of Michigan RN upon hire
Nurse Assessment Coordinator Registered (RNAC) within 1 year
BLS through AHA or ARC within 60 days
Preferred
Bachelors in Nursing
Experience in sub acute care and knowledge of Prospective Payment System (PPS)
