Position Summary:
The Geriatrician will diagnose treat and manage the unique healthcare needs of elderly patients. They will build a program with a multidisciplinary team to provide optimal outpatient management for geriatric patients while keeping them at home and healthy. The ideal candidate will also have expertise in chronic disease management medication optimization comprehensive geriatric assessments and clinical leadership.
Key Responsibilities:
Provide primary and consultative care for elderly patients including preventive care and chronic disease management.
Conduct comprehensive geriatric assessments and develop individualized care plans.
Coordinate with interdisciplinary teams including nurses social workers and specialists.
Manage polypharmacy and optimize medication regimens.
Educate patients and families on agingrelated health concerns and treatment options.
Participate in quality improvement initiatives and patient safety programs.
Qualifications:
MD or DO degree with completion of an accredited residency program.
Valid medical license in West Virginia
Strong interpersonal and clinical skills.
Experience in geriatrics longterm care or palliative care preferred.
