What you bring to SBHG(qualifications):
Associate Degree plus 2 years of experience in a social service field OR
Bachelors degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field.
Valid California Drivers License.
Must be Bilingual Spanish Speaking
How you will make a difference(job overview):
The Case Manager position has primary responsibility for collaborating with the treatment team to facilitate the appropriate discharge and transfer of clients to the appropriate services and level of care. This position is responsible for participating in the treatment planning process that begins from the initial assessment and unfolds throughout the course of treatment in the program through collaboration with the client and the BHUCC treatment team. The case manager participates in interdisciplinary meetings collaboration and communication with outside partners and resources and with preparing clients for program transfer and discharge. Confirms delivery of all ordered medication(s) from pharmacy on shift worked. Responsible for writing summary on clients as assigned. Makes written or oral shift assignments and plans organizes and directs the nursing activities of the unit (when needed and assigned) to assure quality care delivery.

Clients may stay up to 24 hours
BHUCC equipped to serve 12 adults (ages 18 and up) and 6 adolescent (ages 13 to 17

Grade spread 20.90 31.36(In accordance with California law the salary grade for this position is listed above. Starting pay within the grade is determined based on experience internal equity and other factors permitted by law.
