Come grow with us! We are hiring a Clinical Care Specialist to join our Crestone Recovery team of professionals at 41 Montebello Road in Pueblo.

What Youd Be Doing

As a Clinical Care Specialist with Crestone Recovery services you would play a central role in helping our clients access services and resources and take the next steps to wellness. You would use your professional case management skills to assist clients in managing and overcoming their symptoms and difficulties to improve daytoday functioning support other members of the treatment team and collaborate with professionals within and outside our agency to assure that clients receive the very best care available.

Specifically the Clinical Care Specialist

  • Organizing services with the Program Supervisor/Criminal Justice Services;
  • Identifying stressors and precursors to crises;
  • Identifying coping mechanisms and natural supports that may be available;
  • Provide education regarding cooccurring diagnoses;
  • Enhancement of problemsolving skills;
  • Providing a consistent support and availability until the consumer has achieved a higher level of functioning;
  • Linking the consumer with community resources;
  • Helping the consumer to make and keep scheduled appointments with medical doctors;
  • Providing/arranging transportation for scheduled appointments;
  • Providing support to reduce usage of/divert from more restrictive treatment settings or jail;
  • Keeping accurate complete and uptodate records with 100 compliance with company standards;
  • Maintaining competency and proficiency with the agencys Electronic Health Record;
  • Performs other jobrelated duties assigned by the director or his/her designee.
  • Physical requirements include the following:
  • Frequently stays stationary for prolonged period.
  • Frequently moves within and between facilities.
  • Frequently exposed tostressful oremergentsituations.
  • Frequently operatesa computer and other office equipment to record health data.
  • Constantly communicates with clients and others.
  • Occasionally moves equipment and/or materials up to 20 pounds.
  • Occasionally drives between Health Solutions facilities or for other assigned duties.

Qualifications

What Were Looking ForThe MustHaves

  • Bachelors degree in social work psychology criminal justice or a closely related field
  • Experience in case management/community systems navigation particularly with the criminal justice and/or mental health systems.
  • Experience working with individuals with substance use/cooccurring disorders services
  • Experience working with persons diagnosed with severe & persistent mental illness
  • Exceptional flexibility time management and documentation skills
  • Must be a selfstarter and selfmotivator
  • Computer proficiency
  • Colorado drivers license a good driving record and access to transportation during work hours
