Were looking for those that can help enable clients to live independently in accommodation suitable to their needs and to maintain their home in the community. Youll help them to successfully build on skills that will support the client to access work training or education and communitybased activities. Further to this youll support clients to maintain and sustain financial inclusion promoting and maintaining healthy lifestyles and engaging in meaningful activities.
In this role you will be responsible for
Use techniques such as coaching and strengthsbased approaches to enable clients to develop emotional resilience overcome barriers and take control of their lives.
Take a lead on liaising with external agencies raising any safeguarding issues in an appropriate and timely manner
Involved in completing the Positive Behaviour plan Support plan Essential profile and Risk assessment for the customers
Workload and client numbers will be dictated by needs of the clients and demand for aspects of the service.
Under direction from the Project Manager selfmanagement coaches outreach will support the project manager to ensure that all aspects of housing management housing related support and enablement support is carried out to the highest level.
Work as part of a team within supported housing and outreach services providing high quality personalised housing related support and enablement support to young adult clients on outreach basis with a diagnosis of ADHD and ASD and outreach clients with substance misuse issues.
We need people who are
Knowledge and understanding of the needs of complex needs and vulnerable adults.
Knowledge of the issues that can impact on the lives of clients: behaviour management understanding diagnoses leaving care and mental health.
Experience of working in a supported housing environment or relevant social care setting.
Experience of working with young people and vulnerable adults with complex support needs. Interview/application form
Experience of working with statutory agencies and voluntary support services.

The ICL team
Our Independent & Community Living teampride themselves on delivering a fantastic service to our customers providing accommodationbased services to adults and young people across Greater Manchester and striving to make a real difference in peoples lives!
Forging positive and supportive relationships with our customers is central to what we do none more than in our Community & Independent Living team where we provide supported housing and independent living schemes for customers of all ages and with a variety of support needs. Whether our team are onsite 24hours a day supporting young people living with Autism and ADHD working at one of our fantastic independent living schemes for older people or based at our specialist service for people with memory loss and dementia we ensure our customers are at the heart of the services we provide supporting them to develop and maintain their independence.
As well as working to support our customers our team also makes sure the services we run are great places to live and really call home. Safety is always a priority and our teams take a proactive role in making sure homes are always safe secure and well managed. Its a great feeling for the team knowing they can help to make services better for our customers!
