JOB DESCRIPTION
Primary Care Physicians (PCPs) offer a comprehensive range of primary care services which include diagnosing illnesses providing treatment coordinating care offering preventative care and ensuring patients receive the best possible health maintenance. They act as key figures in the patientcentered medical home model and work in collaboration with the care team to deliver highquality healthcare to every patient.

DUTIES
- Actively participate in IDT communicating participant changes collaborating on care planning decisions and coordinating 24hour care delivery.
- Help maintain care of participants in the nursing home making regular visits as dictated by participant needs.
- Take part in rotating night and weekend calls.
- Diagnose and treat a wide range of acute and severe/chronic illnesses.
- Ordering and interpreting diagnostic tests.
- Provide annual checkups and routine health screenings.
- Counsel on lifestyle changes to prevent diseases.
- Coordinate care with other physicians and specialists as needed.
- Provide ageappropriate immunizations.
- Bone and joint care.
- Care for patients of all ages from newborns to elderly.
- Care for chronic conditions such as diabetes asthma and heart disease.
- Ear nose and throat care.
- Emergency care.
- Eyecare.
- Minor surgical procedures.
- Mental and behavioral healthcare.
- Reproductive system care for men and women.
- And other duties as required.
EDUCATION AND QUALIFICATION REQUIREMENTS
- BE/BC Certification.
- Masters degree/Doctorate Degree.
- State CDS Certification and DEA License.
- The appropriate license to work in their state/county.
- Valid unencumbered California Medical License without restrictions.
- Two 2 years minimum of acute Primary Care Physician Experience.
- Board certified in Internal Medicine Family Medicine or Geriatrics.
- ACLS Certification from the American Heart Association.
- Proof of vaccination prior to employment is Required.
- Valid State Drivers License with a clean DMV record.
- Copy of recent Vehicle Insurance Required.
- Extensive knowledge of medical and diagnostic procedures.
- Proficiency in electronic health records (EHR) systems (Preferred).
- Able to work independently and collaboratively with other health providers.
- Highlevel interpersonal skills and the ability to connect with participants during stressful experiences.
- Fluent in English.
