Job Description
Pay:$22.50
Nature of Work:
The Case Manager is responsible for the provision of case management services to children enrolled in Regional Health Home. Referrals come from Steuben County Department of Community Services as well as several other sources throughout the community. All work will be carried out in accordance with wellestablished protocols for the delivery of case management services.
Minimum Qualifications:
- Candidates will have a Bachelors degree preferred in Social Work Psychology or a related Human Service field. Experience is preferred but not required.
- Experience in working within a multidisciplinary team.
- Must meet the requirements of the Mental Health Regulations regarding a State Central Register Clearance regarding history of abuse / maltreatment of children. Must meet insurability guidelines and pass Motor Vehicle Record clearance.
