Job Description
WellStone is North Alabamas most comprehensive behavioral healthcare provider serving over 13000 persons annually throughout Northern Alabama with offices in Huntsville and Cullman. We are a nonprofit community organization that offers a wide range of services for every phase of life from children to older adults.
In this rewarding position you will work directly with clients in our WellStone Emergency Services facility (WES). Providing resources for care assisting the therapeutic team and encouraging healthy choices as your clients transition to outpatient therapy and treatment. This position is Part time weekends
What were looking for:
- Bachelors Degree in social sciences field
- Excellent 5 year driving record & reliable transportation
- Excellent Oral and Written Communication Skills
- State of Alabama Adult Case Management Training
- Knowledge of Community Layout (Madison County Area)
- Knowledge of Community Agencies and Resources
- Personal Automobile Liability Insurance in the amounts of $100k $300k $50K
What youll be doing;
- Provide inpatient case management services.
- Provide direct services to mentally ill clients to address needs that may have been unmet as well as information about the continuum of care;
- Provide followup progress as per need of the client to treatment team members;
- Provide Basic Living Skills training when necessary
- Assist clients with obtaining medication
- Monitor medication compliance maintain awareness of common side effects and ensure the client is taking medication as directed
- Provide traditional case management services as well as collateral services such as transportation to appointments referral agencies etc.;
- Maintain highquality client documentation and records management through electronic charting.
- Monitor the care and services clients receive in a residential setting
- Develop and implement an Individual Service Plan which consists of goals resources andinterventions;
- Revise clients goals once met or progress as they occur
- Assist mentally ill clients through crises and arrange for such assistance by other professional or personal caregivers.
- Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness
