Job Description

Position Overview:

The NonMedical Case Manager is responsible for assessing and coordinating care for program participants ensuring that they receive the necessary services to improve their physical and mental wellness. This role involves collaborating closely with program participants their support networks and healthcare professionals to develop and implement tailored care plans. The NonMedical Case Manager also plays a key role in monitoring program participant progress providing counseling or intervention when needed and performing outreach activities as part of the Ending the HIV Epidemic (EHE) or Minority AIDS and/or (MAI) initiative.

Key Responsibilities:

Client Assessment & Care Planning:

Assess program participants physical and mental wellness needs preferences and abilities.

Develop tailored care plans based on these assessments.

Collaborate with program participants family members friends and healthcare professionals to implement care plans.

Counseling & Support:

Listen to program participants concerns and provide counseling or intervention as needed.

Offer support and resources to help program participants manage their conditions and improve their overall wellbeing.

Progress Monitoring & Evaluation:

Record program participants progress including charting referrals home visits and other notable interactions.

Periodically evaluate program participants progress and adjust care plans as necessary to improve outcomes.

Follow up with discharged program participants to ensure satisfaction with services and monitor their ongoing physical and mental health.

Service Coordination & Delivery:

Develop plans for the evaluation treatment and care of program participants who require assistance due to age illness disability or other challenges.

Assess program participants social service needs and coordinate the delivery of necessary services.

Ensure that services are obtained according to the case plan and monitor progress to ensure a beneficial impact.

Vocational Support & Community Referrals:

Evaluate program participants background education and training to help them develop realistic vocational goals.

Refer program participants to community services that assist with job placement competencies such as interviewing organizational and time management skills and networking.

Outreach & Community Engagement:

Perform outreach activities as part of the Ending the HIV Epidemic (EHE) or Minority AIDS (MAI) initiative.

Work to increase the availability and utilization of agencyspecific services within the community.

The individual in this position will play a crucial role in supporting program participants creating programming coordinating services and contributing to the success of the Ryan White Part A MAI and Ending the HIV Epidemic initiative.

QUALIFICATIONS: To perform this job successfully an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE: Bachelors degree in Social Work Psychology Human Services or a related field. Masters degree preferred.

Experience in case management counseling or a related field is preferred.

Strong understanding of the challenges faced by individuals with HIV chronic illnesses or disabilities.

Excellent communication interpersonal and organizational skills.

Ability to collaborate with program participants their support networks and healthcare professionals.

Commitment to providing compassionate clientcentered care.

LANGUAGE SKILLS: Ability to read and comprehend instructions correspondence and memos. Ability to write correspondence. Ability to effectively present information in oneonone and small group situations to customers clients and other employees of the organization. Bilingual Preferred

MATHEMATICAL SKILLS: Ability to add and subtract twodigit numbers and to multiply and divide with 10s and 100s. Ability to perform these operations using units of American money and weight measurement volume and distance. Development of Standards. Analyzing Information. Dealing with Complexity. Data Entry Reporting Research Results. Confidentiality. Meticulous.

REASONING ABILITY: Ability to apply common sense understanding to conduct detailed but uninvolved written or oral instructions. Ability to deal with problems involving concrete variables in standardized situations.

COMPUTER SKILLS: To perform this job successfully an individual should be proficient with MS Office: Excel and Word Outlook database programs provide enterprise internet. Online HR and payroll access and Program Participant Grocery Choice Program.
