About Us:

We are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents lives in a rewarding setting.
Position Summary:
As a PRN Physical Therapist you will evaluate and treat residents to improve their mobility strength and functional independence. You will work collaboratively with a multidisciplinary team to deliver highquality individualized care.
Key Responsibilities:
- Perform comprehensive evaluations of residents physical abilities and needs.
- Develop and implement personalized treatment plans to improve strength mobility balance and overall function.
- Provide therapeutic interventions including exercises manual therapy and training on assistive devices.
- Monitor residents progress and modify therapy plans as needed to achieve goals.
- Collaborate with an interdisciplinary team to ensure holistic care.
- Educate residents and their families on therapy goals exercises and strategies for continued improvement.
- Accurately document evaluations treatment sessions progress notes and discharge plans in compliance with regulatory and facility standards.
- Participate in quality improvement initiatives and contribute to the rehabilitation departments success.
Qualifications:
- Doctorate or Bachelors/Masters degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in the state of Colorado.
- Experience in skilled nursing or rehabilitation settings is preferred.
- Strong communication and teamwork skills with a residentcentered approach.
- Ability to work independently and adapt to a flexible PRN schedule.
- Dedication to providing compassionate and effective therapy services.
