Description

Position Summary:

The Street Outreach Coordinator is responsible for overseeing and implementing outreach efforts to engage individuals at risk of violence homelessness or other crises. This role ensures that outreach teams effectively connect with the community provide resources and offer support services. The coordinator will also maintain strong relationships with local organizations law enforcement and community stakeholders to foster a collaborative approach to violence prevention and crisis intervention.

Key Responsibilities:

Outreach & Community Engagement:

  • Develop and implement outreach strategies to connect with atrisk individuals in targeted communities.
  • Supervise and support outreach workers in conducting streetlevel engagement crisis intervention CPIC Chicago Department of Public Health and community support efforts.
  • Establish and maintain relationships with community partners local businesses law enforcement and service providers.
  • Coordinate and participate in community events mediation efforts and violence prevention initiatives.

Case Management & Resource Coordination:

  • Assess individuals needs and provide referrals to social services mental health support housing assistance and employment programs.
  • Track and follow up with individuals engaged through outreach to ensure they receive ongoing support.
  • Work with program leadership to identify gaps in services and develop strategies to address community needs.

Program Oversight & Compliance:

  • Maintain accurate records of outreach efforts participant interactions and service referrals.
  • Ensure data collection and reporting align with grant requirements and organizational policies.
  • Provide training guidance and professional development for outreach workers.
  • Collaborate with leadership to evaluate program impact and effectiveness.

Crisis Intervention & Conflict Mediation:

  • Respond to community incidents including violence and emergencies to provide support and deescalation.
  • Work with outreach teams to mediate conflicts and prevent retaliatory violence.

Assist in implementing traumainformed care practices and harm reduction strategies.

Qualifications

Qualifications & Requirements:

  • Education: Associate degree and or bachelors degree in social work public health and or a related field (preferred) or equivalent experience.
  • Experience: Minimum of 23 years in community outreach violence prevention crisis intervention or related work.
  • Strong understanding of traumainformed care conflict mediation and restorative practices.
  • Ability to build trust and rapport with individuals from diverse backgrounds.
  • Excellent verbal and written communication skills.
  • Ability to work flexible hours including evenings and weekends as needed.
  • Valid drivers license and reliable transportation preferred.

Preferred Skills:

  • Experience managing a team of outreach workers.
  • Familiarity with data tracking and reporting software (e.g. Salesforce Excel Microsoft Team/Outlook).
  • Knowledge of community resources social services and intervention programs.
