In this position the social worker provides comprehensive psychosocial assessment management and support to at risk patients and families on the neuroscience units at Upstate Medical University Hospital. Knowledge of social determinants population health and drivers of utilization. Duties will include consultation through assessment counselling intervention planning and referrals to meet the psychosocial needs of patients and families. Candidate shall learn to initiate and implement advance directives. In addition the social worker will actively manager patients through collaboration coordination monitoring evaluating options and identifying required services to meet individuals health needs. The social worker rounds on their assigned patients with the multidisciplinary team. Additionally the social worker will participate in quality meetings on their assigned unit. Candidate must possess communication skills have knowledge of available community resources document all interventions; participate in departmental meetings/inservices education and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and backup responsibilities.
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W. L.M.S.W. L.C.S.W. or eligible 3 years relevant postmasters experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2 position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
MondayFriday 8:30a5:00p with occasional on call/holiday coverage on a rotating basis to meet operational needs.
