Responsibilities:

  • Apply appropriate tests and assist the physician in evaluating and assessing the clients level of functioning.
  • Assist in developing the plan of care.
  • Treat clients to relieve pain develop and restore function and maintain maximum performance using appropriate physical therapy treatments modalities and procedures as stated in the physical therapy plan of care.
  • Observe record and report the clients reaction to treatment and any changes in the clients condition to the physician Director of Nurses and Case Manager.
  • Educate the client/caregiver and health care team in the phases of physical therapy in which they may assist the client in the use of prosthetic and orthotic devices and durable medical equipment as appropriate.
  • Assess and regularly reevaluate the needs of the client. Develops implements and revises the clients plan of care.
  • Develop prepare and maintain individualized client care progress records with accuracy timeliness and according to agency policy and procedures.
  • Participate in staff meetings inservice programs and Utilization reviews.


Qualifications:

  • Must have Certified Licensure for Physical Therapy in the state of MN.
  • Must have Drivers License auto insurance and reliable vehicle.
  • Willing to travel to patients home to perform evaluations/visits
