Job Duties:
Coordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients.
- Provides discharge information.
- Partners with providers to ensure a smooth transition at discharge to home or other facilities as required.
- Ensures additional resources are made available for the patient as needed.
- Effectively collaborates with other healthcare providers and nonGeisinger employees in coordinating and implementing the patients plan of care.
- Maintains systems and processes to assure timely followup and surveillance of a defined group of patients.
- Serves as liaison with clinical and administrative personnel.
- Establishes point of contact for referring providers to create immediate access to provider templates.
- Develops and implements efficient patient navigational procedures.
- Investigates patient concerns including billing appointment access perception of care physician issues and communication problems.
- Develops maintains and audits standard operating procedures of patient access and utilization process.
- May provide administrative support as needed.
