Intermountain Centers and its statewide affiliates Community Partners Integrated Healthcare Pinal Hispanic Council Intermountain Health Center Intermountain Foster Care Behavioral Consultation Services and MHRI Housing are currently recruiting careerminded individuals interested in opportunities within the largest adult and child service continuum in Arizona.
GENERAL SUMMARY:The Community Counselor provides strengthbased interventions to build self regulation skills and develops positive coping strategies for anger impulse control depression anxiety and stress conflict resolution and general self management. Services will be provided to member and/or family in a community educational or homebased setting. Care coordination and Child and Family Team (CFT) facilitation skills will also be required and provided as needed based on the needs of the member. Community counselor may also assist in the facilitation of group counseling sessions on a rotating basis.
JOB RESPONSIBILITIES:
- Provides direct family centered clinical services to members as well as care coordination to meet the needs of the member and employee productivity standards. Services include individual family and group skills building interventions and counseling services to address a variety of complex issues such as behavioral challenges parenting skills family systems issues anger management and substance abuse interventions.
- Prepares written diagnostic assessments assessing service recipients from multiple perspectives including family functioning psychological/emotional social academic medical behavioral substance abuse skills strengths barriers to selfsufficiency treatment history and others and utilizing various assessment instruments.
- Facilitates Child and Family Team (CFT) meetings. Collaborates and coordinates with other service providers system partners and professionals such as attorneys school personnel social workers and probation officers.
- Interpretand followtreatmentplan from otherproviderswhenclinicallyappropriate.
- Respondstocrisissituationsduring theworkdayasneeded.
- Documentscaseactivitiesforboth clinicaland billingpurposesin accordancewithagencyand AHCCCSstandards.
- Workswith ClinicalSupervisortoincreaseeffectivenessoftherapeuticintervention.
- Performsotherdutiesasassignedornecessaryasthey relate tothegeneralnatureoftheposition.
QUALIFICATIONS:
- EducationAssociatesdegreepreferredhigh schooldiplomaorGED required.
- ExperienceMinimum4yearsofdirectmembercareexperienceinBehavioralHealth.
- SpecialJobRequirementOwn reliable transportation; mileage reimbursement provided.
REGULATORY:
- Minimum 18 years of age.
- Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
- CPR First Aid & AED certification if required (must possess upon hire and maintain throughout employment).
- Valid Drivers License 39month Motor Vehicle Report proof of vehicle registration and liability insurance that meet company insurance requirements if required.
- Negative TB test result if required (Employer provides).
