FUNCTIONAL JOB DESCRIPTION

COMMUNITY TRAINING SPECIALIST (DAY TIME AND OVERNIGHT)/RELIEF FOR DEAF RESIDENTIAL PROGRAMS

Reports To: Community Living Supervisor or House Manager Department: Residential

Job Summary:

The Community Training Specialist is responsible for implementing individual service plans and for facilitating as normal an atmosphere and routine life as is possible in the community residence for individuals with developmental and/or physical disabilities.

Qualification/Education/Experience:

  1. Bachelors degree in Education Special Education Social Work or related field and/or H.S. diploma and related experience.
  2. Possession of a valid Massachusetts Drivers License and safe driving record in accordance with WORK INC policy.
  3. Fluent in ASL (for deaf programs.

Essential Functions:

  1. Ability to effectively communicate with individuals served.
  2. Helps people grow through their life experiences.
  3. Creates opportunities for people to form relationships with nondisabled individuals
  4. Assists individuals with ADLs and maintaining proper hygiene.
  5. Assists and engages individuals with meal preparation.
  6. Engages individuals in meaningful social activities in their home environment.
  7. Teaches independent living skills
  8. Provides resources and opportunities for people to express their ethnic and cultural background
  9. Follows through on all medical recommendations to ensure a healthy lifestyle.
  10. Follows existing treatment programs.
  11. Follows emergency protocols including conducting fire drills as directed.
  12. Ensures the health and safety of each individual.
  13. Helps individuals in crisis situations.
  14. Helps people become socially integrated into their communities.
  15. Creates an environment that encourages people to express their wishes dreams and desires.
  16. Helps individuals achieve their personal goals.
  17. Collects necessary data to demonstrate the effectiveness of training programs.
  18. Assists people in accessing community resources.
  19. Participates in the development of individual service plans.
  20. Attends inservice training sessions and staff meetings.
  21. Meets regularly for supervision.
  22. Cleans and maintains the facility and grounds as assigned including necessary snow

removal.

  1. Provides transportation to and from day programs and other destinations as needed.
  2. Maintain all required certifications and trainings outlined in the Residential Employee

manual.

  1. Required to sign in and out for every shift.
  2. Relief staff will receive instructions and direction from the regular direct care staff while on shift in the absence of the program supervisor.

Mathematical Skills:

  1. Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals.
  2. Ability to compute rate ratio and percent ability to create and interpret graphs.

Reasoning Ability:

  1. Ability to define problems collect data establish facts and draw valid conclusions.
  2. Ability to interpret an extensive variety of technical data in mathematical or diagram form and deal with several abstract and concrete variables.

Communication Demands:

  1. The ability to read write analyze and interpret journals financial reports and legal documents.
  2. The ability to respond to common inquiries of complaints.
  3. The ability to effectively present information to employees program participants and members of the community.
  4. Ability to effectively communicate with families/guardians and funding agencies and ancillary services.

Environmental Demands:

The work is performed in a nonsmoking environment. The work is normally performed in a temperature controlled zone however there are exceptions. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. General requirements:

1. Good standing balance defined as the ability to stand and walk without a loss of

balance.

2. Full upper extremity range of motion defined as the ability to obtain:

Full shoulder flexion extension abduction and rotation.

Full elbow flexion extension supination and pronation.

Full wrist flexion extension radial and ulnar deviation.

3 Normal upper extremity strength defined as the ability to move the specified

body part through a full range of motion against maximum resistance.

4. Full lower extremity range of motion defined as the ability to obtain:

Full hip flexion extension abduction adduction and rotation.

Full knee flexion and extension.

Full ankle flexion and extension.

5. Normal lower extremity strength defined as the ability to move the specified

body part through a full range of motion against maximum resistance.

6. Full trunk range of motion as defined by the ability to obtain full flexion and

extension rotation and side bending.

7. Full cervical range of motion as defined by the ability to obtain full flexion and

extension lateral flexion and rotation.

*Frequent: Reaching walking climbing stairs pushing pulling lifting

*Occasional: Bending Stooping

*Definitions: continuous 66 to 100 of the time

frequent 31 to 65 of the time

occasional 10 to 30 of the time

rarely less than 10 of the time
