Job Description
Licensed Vocational Nurse / LVN
Annual Salary: $69k
Schedule:Monday Friday 8:00a 5:00p
This is a role supporting RNs in the field and office with patient visits and charting

As a Home Health/Hospice Licensed Vocational Nurse heres a bigpicture view of what youll do:
- Provide homebased care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN
- Provide support to field RNs to include field visits and support in office charting
- Work with a team of physicians RNs CNAs and aides to deliver quality patient care
- Take patient vitals do assessments document progress and report changes to RN supervisor
- Assist RN in carrying out patients plan of care and teaching patient selfcare techniques
- Educate family on the patients plan of care medication and home safety
- Provides appropriate pain/symptom management. Evaluate patients response to treatments/medications
- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
A few musthaves for Home Health/Hospice Licensed Vocational Nurses:
- Graduate of an accredited vocational nursing program and active LVN license in CA
- Twelve 12 months experience in an acute care environment
- One 1 year of hospice care experience preferred
- Minimum of 1200 hours of LVN experience ideally in home healthcare or completion of offices preceptor program
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing strong communication skills and compassion
