Family Home Provider Job Description

Policy: All individual contractors providing Residential Level II Family Home Provider services for Zoom Group shall adhere to the Supports for Community Living Regulations and Zoom Groups Policies and Procedures.

The Family Home Provider will provide Residential Level II services in a home not owned or leased by Zoom Group where the SCL resident requires 24hour support and resides and receives support in the

familyoccupied home. A family providing Residential support includes the resident in the familys household routines provides training and supervision and ensures the residents needs are met per their Plan of Care and the Supports for Community Living Policy Manual.

Primary Functions and Responsibilities:

1. The duties of a Family Home Provider contractor for the SCL Waiver include but are not limited to:

A. Providing individually tailored support for the resident that assists with the acquisition retention or improvement in skills related to living in the community.

B. Helping the resident with adaptive skill development and assisting the person to reside in the most integrated setting appropriate to his/her needs.

C. Providing protective oversight and supervision per the residents Person Centered Plan.

D. Providing transportation or arranging transportation between the home other service sites and community locations.

E. Provide access to medical and health care services integral to meeting the residents daily needs.

F. Promoting increased independence.

G. Assisting with activities of daily living including bathing dressing toileting transferring meals or maintaining continence.

H. Supporting residents in community inclusion adult education supports Social and leisure development and personal assistance.

I. Provide oncall support with at least one facetoface contact per day in the home to promote increased independence as identified in the Plan of Care developed by the personcentered team.

J. Provides support up to 24 hours a day.

K. Monitoring changes to support needs/status and make changes to residential care provided to meet the residents needs. If changes are anticipated to

be chronic (lasting more than three months) FHP should report this to the LifeStyleZ Manager to pursue exceptional support approval through DDID. FHP must report to the LifeStyleZ Manager when the resident returns to the previous status.

L. Tailoring supports to meet the level and area of need of the resident based on the supports Intensity Scale (SIS) Assessment Health Risk Screening Tool (HRST) and approved individual Plan of Care (POC).

M. Ensuring the resident is free of abuse neglect and exploitation.

N. Ensuring resident rights are respected and intact in all settings.

O. Ensuring the resident is healthy and safe.

P. Ensuring residents have an integrated lifestyle.

Q. Implementing a personcentered plan in the home.

R. Ensuring residents are provided a smokefree odor/chemicalfree environment in the home.

S. Ensuring the home is equipped with functioning smoke detectors and fire extinguishers and have an adequate fire safety plan and emergency/disaster plan to include:

i.means to alert residents in emergency situations according to their level of disability.

ii.proof of regularly scheduled/conducted drills.

T. Ensuring a supply of hot and cold water in the home complying with the safety limits established in the residents POC.

U. Ensuring residents home is free of deadly weapons. If deadly weapons are present a safety plan must be established and followed to ensure the residents wellbeing. The deadly weapons are to be doublelocked and ammunition is stored separately and doublelocked.

V. Ensuring safe storage of standard household items such as household cleaning products.

W. Ensuring the residents nutritional needs are met.

X. Ensuring medication prescriptions and MARS are accurate and up to date and medications are stored in a locked area and controlled substances stored double locked.

Y. Ensuring appropriate documentation of PRN medication usage and effectiveness.

Z. Supports the resident to help manage spending money and necessary document expenses and provide receipts as required by policy.

AA. Participates as a member of the residents personcentered team if requested by the resident.

BB. Provide a bedroom where the resident does not share a room with anyone unless approved by the residents team. The bedroom needs to be lockable and a key provided to the resident unless the residents team deems it is a Health safety or welfare issue. Provide a bed with a box spring a clean and comfortable mattress and clean linens.

CC. Notify Zoom Group about any changes to the living arrangements including another funded individual from another agency who is moving in someone over the age of 18 moving in or someone already in the home turns 18 so if any actions by Zoom Group are needed it can be addressed.

DD. Allow and encourage residents to spend time with natural supports as appropriate and as decided by the resident or their guardian.

EE. Check the email associated with Zoom Group daily to ensure important information is not missed.

2. If you as a Family Home Provider ever pay anyone to assist the resident in any way regarded as respite care

A. That person must be a Family Home Provider with Zoom Group and in good standing.

B. You are responsible for paying that individual from your stipend.

C. Proper advance notification must be communicated with Zoom Group to ensure that all team members are aware and in agreement.

3. Documentation tasks related to service provision shall include but are not limited to:

A. Daily attendance completed on the service date shall include:

i.date of the service

ii.location of service

iii.signature and title of the individual providing the service

iv.date the entry was made in the record

B. Detailed monthly summary note due by the 5th of the month for the prior month shall include:

i.The month and year for the period covered by the note

ii.The analysis of the progress towards the residents outcome

iii.A plan to achieve an outcome(s)

iv.Pertinent information regarding the residents life

v.The signature and title of the person writing the note

vi.The date the note was written

C. Physician consult forms to record pertinent information for all medical appointments are sent to the LifeStyleZ Manager within 24 hours.

D. Filling out logs related to tracking the health and safety needs of the resident

E. According to their personcentered service plan daily

F. Daily MARS

G. Fire and Severe Weather Drills

4. Incident Reporting

A. Any incident that impacts the residents Health Safety Welfare or Lifestyle Choices must be reported to Zoom Group immediately or as soon as the resident is safe.

B. FHP must report all medication errors (missed medication wrong medication wrong dose wrong person wrong time given wrong route wrong documentation) immediately to be documented on a medication error report form.

Minimum Requirements:

Must be 18 years old or older with a high school diploma or GED and meet all applicable DDID personnel and training requirements.

Must be 21 years old or older if you do not possess a high school diploma or GED and meet all applicable DDID personnel and training requirements.

Must have a valid social security card or valid work permit if not a citizen of the United States.

Must have the ability to communicate effectively with the residents family understand and carry out instructions and perform required documentation.

Must have a valid class D state of KY drivers license with no major traffic violations and current mandatory insurance.

Must have a state police criminal record check for any state resided in during the previous year. If there is a prior conviction of any offense delineated in KRS Chapter 17.165 13 or a prior felony conviction the applicant shall not be employed.

Must speak write problem solve and maintain records clearly and concisely.

Possess the ability to handle multiple tasks be computer literate and have reliable transportation.

Demonstrate competence and knowledge on topics required to safely support the resident as described in the residents personcentered plan.

Ability to understand and commit to Zoom Groups Mission and Values.
