Summary Job Description:

A bilingual Licensed Clinical Social Worker Licensed Professional Counselor or Licensed Psychologist is sought to work as a primary care team member and provide shortterm clinical behavioral health consultation services in a fastpaced setting. Services must be provided in a manner that upholds the health centers mission and quality standards results in better patient health outcomes and provider and patient satisfaction and strengthens the operational and fiscal capacity of the health center. The Behavioral Health Consultant reports to the Behavioral Health Consultant Network Supervisor.

Essential Duties and Responsibilities: Behavioral Health Consultation in the context of primary care

  • Consultation with primary care providers to improve patient care enhance understanding of patients provide support for treatment planning and assist in the implementation and monitoring of evidencedbased assessments and interventions.
  • Individual couples family and group consultation including:
    • Functional and strengthbased assessment and diagnosis.
    • Psychoeducation for patients and their support systems.
    • Medication adherence counseling and disease self management counseling.
    • Motivational Interviewing to develop behavioral strategies aimed at symptom reduction.
    • Brief problemsolving cognitive intervention aimed at modifying negative thinking and promoting selfefficacy.
    • SelfCare Plan development and skills training to facilitate disease management improved coping distress tolerance stress reduction and relaxation; and
    • Substance use/abuse evaluation identification of maladaptive coping strategies and development of harm reduction strategies.
Coordination of Care Provide consultation to and coordinate care of patients with health center primary care staff. Identify refer and advocate for patients needing specialty behavioral health service and other services as needed.
Crisis Intervention Be immediately available to other health center staff during working hours in the event of a psychiatric emergency. Participate as a member of the health center staff in the event of any other kind of emergency.
Population Management Identify population health issues and work with other providers to develop comprehensive strategies to address them.
Charting Complete all charting as needed and required by contracted funding sources: including assessments SOAP notes and billing.

PHMC Compliance Responsibilities:

  • Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct Whistle Blowers and Conflict of Interest Policies
  • Keeps abreast of all pertinent federal state and PHMC regulations laws and policies as they presently exist and as they change or are modified
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job
  • Comply with Department of Public Health (DPH) The Joint Commission and other accreditation and regulatory agencies standards
  • Adhere to all PHMC Policies and Procedures
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module

Job Requirements:

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below must be representative of the knowledge skills minimum education training licensure experience and/or ability required. Skills:

  • Oral and written fluency in Spanish required.
  • License in Clinical Social Worker Professional Counseling or Psychology.
  • Excellent engagement assessment and clinical skills.
  • Excellent recordkeeping and organizational skills.
  • Excellent written and communication skills.
  • Able to utilize DSMV for clinical diagnostic assessment.

EXPERIENCE:

  • Experience working in a medical setting.
  • Medical literacy
  • Experience working in behavioral medicine and health psychology.
  • Experience working as a member of an interdisciplinary team.
  • Comfort with evidencedbased protocols.
  • Experience with Cognitive Behavioral and Motivational Interviewing techniques.
  • Comfort working with diverse populations (i.e. race ethnicity diagnoses age ranges)

Education:

  • Licensed Clinical Social Worker (in Pennsylvania) or
  • Licensed Professional Counselor (in Pennsylvania)
  • Licensed Psychologist (in Pennsylvania)

Physical Demands:

Position requires standing 2/3 of the time walking 2/3 of the time requires sitting under 1/3 of the time use of hands to finger handle or feel 2/3 of the time reach with hands and arms under 1/3 of the time stoop kneel crouch or crawl under 1/3 of the time talk or hear over 2/3 of the time. Position requires lifting up to 10 lbs. up to 1/3 of the time.

Work Environment:

Moderate noise (examples: business office with computers and printers light traffic). Exposure to blood borne pathogens that requires use of personal protective equipment.
