Job Description

Job Title: Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker
Department: Med Ops
Reports to: Practice Manager
Location:With a mix of in-person and virtual care this position requires travel between two (2) clinic locations on a weekly basis; Fair Oaks (IH-10 and Ralph Fair Road) and Schertz (IH-35 and Schertz Parkway). You will not typically be traveling between locations on the same day.

The Role:
The Licensed Professional Counselor / Licensed Clinical Social Worker is a key member of the Behavioral Health Collaborative Care team embedded in the Primary Care setting and will work with the Primary Care team and Psychiatric Consultant. Responsibilities will include supporting and coordinating the mental health care of patients including but not limited to individuals couples groups and providing crisis intervention. They will counsel eligible patients who are experiencing depression anxiety addictions substance abuse PTSD family parenting marital problems stress management self-esteem and aging; to promote optimum mental and emotional health.
What Youll Do:
Will support and coordinate the mental health care for retired San Antonio Police and Fire Fighters and their dependents including providing crisis intervention. Will counsel eligible patients who are experiencing depression anxiety addictions substance abuse PTSD family parenting marital problems stress management self-esteem and aging; to promote optimum mental and emotional health. This is a patient population that is extremely rewarding to care for and appreciative; not to mention responsible when it comes to keeping their appointments! Monday - Friday 8:00am - 5:00pm.
