Your Role & Responsibilities

As a Behavioral Health Clinical Team Manager youll be the driving force behind seamless highquality home health services. Your leadership will inspire your team to provide exceptional patient care.

Oversee patient care services including staffing care coordination and referrals.
Ensure timely patient care while adhering to all regulatory guidelines.
Provide clinical oversight supporting and mentoring your team.
Foster strong communication with patients families physicians and providers.
Monitor clinical outcomes update care plans and maintain compliance.
Lead quality improvement initiatives ensuring excellence in care.

What You Bring to Elara Caring

Were looking for strong leaders with a passion for making a difference.

RN License Active state Registered Nurse license.
Education Associates Degree in Nursing (BSN preferred).
Experience 2 years in home health or behavioral health as an RN.
Leadership 1 year of supervisory experience in a healthcare setting.
TechSavvy Familiarity with Workday HomeCare HomeBase and MS Office Suite.
