Job Summary:
Assumes an active role to provide patient care through comprehensive medication reviews and/or targeted diseasespecific interventions.
Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Gathers necessary data to promote patient safety by ensuring that each relevant medication is indicated effective safe and that the patient is adherent to therapy
- Improves patientspecific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy assessing effectiveness and identifying potential interactions or adverse effects.
- Assesses relevant patientspecific medical problems prioritizes medical problems based on urgency and severity and identifies preventive and health maintenance issues as needed. May design a drug therapy plan for patientspecific problems and persuasively justifies recommendations.
- Provides enhanced patient care through detailed counseling on medications and disease states adherence assessment medication cost analysis review of immunization status and communication with patients providers and pharmacy.
- Provides consultation to other health care professionals if requested.
- For Specialty Pharmacy Only: Conducts patient counseling and monitoring as required by the patient management program.
- For Specialty Pharmacy Only: Participates in the quality management program to facilitate continuous process improvement initiatives and quality improvement projects.
- Precept pharmacy students and residents as applicable.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.
EDUCATION
- Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy
EXPERIENCE
PHYSICAL REQUIREMENTS
- Frequent lifting/carrying and pushing/pulling objects weighing 025 lbs.
- Frequent sitting standing walking reaching and repetitive foot/leg and hand/arm movements.
- Frequent use of vision and depth perception for distances near 20 inches or less) and far 20 feet or more) and to identify and distinguish colors.
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms malfunctioning machinery etc.
- Frequent keyboard use/data entry.
- Occasional bending stooping kneeling squatting twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 2550 lbs.
- Rare climbing.
